ABSTRACT

Objective:To provide a reference for the hierarchical management of clinical nurses in top three hospitals, the hierarchical management evaluation system was conducted and evaluated in the present study.Methods:On the context of three-dimensional structure theory of medical quality, the Delphi method was applied to conduct two rounds of correspondence consultation among 23 nursing experts, and then related indexes were established based on the evaluation results and recommendations.Results:An evaluation system for hierarchical management of clinical nurse in top three hospitals was finally constructed, which included 3 first-grade and 19 second-grade indexes. The effective recovery rate of the two rounds of expert questionnaires and authority Cr of two rounds of expert consultation were 100% and 0.915, respectively. The Kendall coefficients of concordance for significance and operability of the first-grade and second-grade indexes were 0.353/0.515 and 0.344/0.469, respectively ( P<0.01). Conclusions:The hierarchical management of clinical nurses in top three hospitals constructed based on three-dimensional structure theory has reliability and feasibility.

ABSTRACT

Objective:To explore the coping trajectory of primary caregivers in patients with permanent enterostomy, and to provide theoretical basis for clinical intervention.Methods:A semi-structured interview was conducted among 20 primary caregivers of patients with permanent enterostomy hospitalized in Qilu Hospital of Shandong University from May to July 2021. The data were analyzed by three-level coding and continuous comparison.Results:The coping trajectory of primary caregivers of permanent enterostomy patients experienced confusion and fatigue period, coping exploration period, acceptance and coexistence period, transformation and transcendence period.Conclusions:Medical staff should give corresponding health education and timely formulate effective intervention measures according to different stages. At the same time, we should effectively integrate external resources to promote the construction of "Hospital-Family-Community" tripartite care model to relieve the care pressure of caregivers.

ABSTRACT

Objective:In order to provide guidance strategies for the establishment and application of self-report inventory in nursing research, we discussed the mental response model regarding tested nurses during its application.Methods:From September to November 2020, based on the grounded theory, we conducted semi-structured interviews for 9 tested nurses, who were from Qilu Hospital of Shandong University and Jinan Central Hospital, and data were analyzed to extract the psychological response topic.Results:A response model for tested nurses, which formed three psychological responses that factored in exam motivation, exam situation and individual will was built.Conclusion:The present study reveals the correlations among exam motivation, exam situation, individual will and psychological responses during the application of self-report inventory on tested nurses, which could provide references for the establishment and application of self-report scale in nursing research.

ABSTRACT

Objective@#To explore and construct the cognitive response model for patients suffering conflicts with nurses, and to provide guidance cues for the prevention and treatment of nurse-patient conflict.@*Methods@#According to the grounded theory approach devoloped by Strauss, a total of 9 patients were observed and interviewed in a semi-structured way, and the collected data was analysed to extract the cognitive response theme.@*Results@#A cognitive response model for patients with nurse-patient conflicts was established, which included three phases, i.e., the demands of expectations, the emotional catharsis, and the introspection of right and wrong.@*Conclusion@#The established congnitive response model confirmes the relationships among environmental factors, patients′ cognitive processes, and patients′ reponse, and can be used to prevent and deal with the conflicts between patients and nurses.

Objective@#To construct evaluation index system of training effectiveness for new recruiting nurses based on the theory of Kirkpatrick Model.@*Methods@#The draft of evaluation index system of training effectiveness for new recruiting nurses was drawn up through literature survey and interview method first, and then it was constructed by using Delphi method and statistical analysis.@*Results@#Through two rounds of expert consultations, the experts′ suggestions were revised and summarized, and then the evaluation index system was constructed, including 4 grade I indexes, 14 grade Ⅱindexes, and 56 grade Ⅲ indexes. The positive coefficients of the two rounds of expert consultation were 96.15% and 100.00%, respectively, with authoritative coefficient of 0.877. The Kendall Harmony Coefficients of importance and operability of grade I, Ⅱ, and Ⅲ indexes were 0.640, 0.378, 0.296, and 0.707, 0.515, 0.416, respectively (P< 0.01).@*Conclusions@#The enthusiasm, authority level and coordination degree of the experts were all at a high level. The evaluation index system of training effectiveness for new recruiting nurses was of high importance and operability.

ABSTRACT

Objective To evaluate the effect of using"metronome"to guide chest compression in the new nurse training of cardiopulmonary resuscitation(CPR). Methods From 2015 to 2016,385 new nurses were enrolled and randomly by random number table divided into four groups based on Solomon four-group design:control group 1(96 nurses),sudy group 1(89 nurses),control group 2(104 nurses) and study group 2(96 nurses),Control group 1 and control group 2 accepted normal training plus "chest compressions operation formula" training,study group 1 and study group 2 got "metronome" guiding based on the frequency training.Control group 1 and study group 1 took a baseline test before the training started,while control group 2 and study group 2 didn't.Four groups took part in an operational test after the seven days of training. Results There was no statistical difference between the baseline test scores of control group 1 and study group 1(P>0.05).After the training,the score of study group 1 was 93.4±3.5, which was significantly higher than 91.6 ± 4.7 of control group 1(t=-2.870, P<0.01).Through covariance analysis,after removing the impact of the baseline test score,the score of study group was statistically significant higher than that of control group 1 (F=7.33, P=0.007).Furthermore,there was no statistically significant interaction between the baseline test score and training method (P>0.05).Meanwhile,after training,study group 2 scores was 93.5±3.5,which was statistically higher than 92.1±4.0 of control group 2(t=2.670,P=0.008).Control group 1 were combined with control 2 to form control group,as well as study group 1 with study group 2 to form study group.A comparison between these two new groups indicates that the new study group surpasses the new control group with lower error frequencies in the position,depth and frequency of external cardiac compression (t=4.701 8, 12.488 9, 11.993 9, P<0.05).In addition,the study group also showed a better performance, when taking into consideration of the region and skills of compression and the observation of complexion,while there was no statistical difference between the error frequencies in these three aspects of these two new groups(P>0.05). Conclusions In the CPR training of new nurses,using a 100 times/min"metronome"can help new nurses grasp the accurate compression rates and depth, improve the overall quality of this procedure,making it a simple and effective training method,which is worthy of popularization.

ABSTRACT

Objective To investigate the influence of intensive heart rate control on inflammatory factor and cardiac function in patients with chronic heart failure. Methods From January 2015 to December 2015 ,a total of 120 CHF patients in New York Heart Association(NYHA)functional classes Ⅱ to Ⅳ were enrolled and randomized into treatment group(n=60)and control group(n=60). All the patients were in stable situation af-ter conventional drug treatment. The patients in treatment group underwent intensive heart rate control for target HR (55~60 beats/min)through adjusting the dose of metoprolol sustained-release tablets. The concentration of C-reac-tion protein(CRP),interleukin-1β(IL-1β),interleukin-6(IL-6)and tumor necrosis factorα(TNF-α)were de-tected before and after 6-month treatment. The resting heart rate and the concentration of brain natriuretic peptide (BNP),left ventricular ejection fracetion(LVEF),left ventricular end diastolic diameter(LVEDD)and left ven-tricular end systolic dimension(LVEDD)were measured at the start and 6-months after treatment. Results After 6-month treatment,the resting heart rate of the patients in the treatment group decreased significantly compared with that of the control group(P<0.001). Inflammatory factors(CRP,IL-1β,IL-6 and TNF-α)levels decreased significantly compared with that of control group (P < 0.05). The echocardiography parameters (LVEDD and LVESD)and the concentration of BNP of the patients in the treatment group decreased significantly(P < 0.05), LVEF of treatment group increased significantly(P < 0.05). Conclusion Intensive heart rate control in patients with chronic heart failure can significantly reduce Inflammatory factor levels and improve the cardiac function.

ABSTRACT

Ferula sinkiangensis K.M. Shen is a kind of endangered national plant medicine in the northern of Xinjiang,China. It mainly contains sesquiterpene coumarins,sulfides,polysaccharides,aromatic compounds and other effective chemical constitu-tions;Chromatography is used to build the fingerprint and to control the medicine quality in most cases. Both raw plant medicine and specific compounds possess pharmacologic activities such as antibacterial,insecticidal,anti-allergic,and antitumor properties. The present paper summarizes the resource distribution,chemical constituents,fingerprints and pharmacological activities of F. sinkian-gensis,to provide reference for its systematic research,effective quality control and efficient development and utilization.

ABSTRACT

Objective To evaluate the effect and mechanism of rt-PA combined with high pressure oxygen (HPO)on cerebral ischemia-reperfusion injury in rats.Methods The model of cerebral ischemia-reperfusion injury was constructed by using middle cerebral artery occlusion.The neurological function score;brain index,water content and infarction volume;SOD;LDH;NOS;MDA;LD;NO and NOS were measured.The protein and mRNA expressions of iNOS,BDNF,p75NTR and TrkB were also detected by RT-PCR and Western blot to evaluate and compare the protective effect of rt-PA combined HPO therapy. Results rt-PA combined HPO could significantly decrease the neurological function score;brain index,water content,and infarction volume;SOD;NOS;MDA;LD;NO and NOS but increase LDH content and the weight of rats,compared with rt-PA.In addition,rt-PA combined HPO could increase BNDF and TrKB expressions and downregulate the expressions of iNOS and p75NTR,compared with rt-PA (P<0.05).Conclusion The rt-PA combined HPO therapy has a greater protective effect than rt-PA therapy and its mechanism might be related to having antioxidant effects, increasing the expressions of BDNF and TrKB,and decreasing the expressions of iNOS and p75NTR.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect of electroacupuncture(EA) at Zhongwan(CV 12) on the energy metabolism along the conception vessel(CV) in volunteers with yang-deficiency constitution,and to explore the relationship of electroacupuncture regulation and body constitution.</p><p><b>METHODS</b>Eighteen volunteers with mild constitution and 18 volunteers with yang-deficiency constitution were collected out of 200 students of Fujian University of TCM by body constitution questionnaire. Skin microcirculatory blood perfusion units (MBPU) at Danzhong (CV 17), Xiawan(CV 10) and Qihai(CV 6) of CV were measured by a laser Doppler flowmetry in the normal condition and after EA stimulation at Zhongwan(CV 12) for 20 min.</p><p><b>RESULTS</b>(1)Before treatment, (1)MBPU values at Danzhong(CV 17), Xiawan(CV 10) and Qihai(CV 6) in the yang-deficiency constitution group were lower than those in the mild constitution group,but there was no statistical significance (both P>0. 05) except Danzhong(CV 17) (P<0. 01). (Z)As for the three acupoints in the mild constitution group, MBPU level of Danzhong(CV 17) was higher than that of Xiawan(CV 10) without statistical significance(P->0. 05),and MBPU values of Danzhong(CV 17) and Xiawan(CV 10) were higher than that of Qihai(CV 6) (both P<0. 01). (3About the three acupoints in the yang-deficiency constitution group, MBPU result of Danzhong(CV 17) was lower than the value of Xiawan(CV 10), but higher compared with Qihai(CV 6)(P<0. 05, P<0. 01). MBPU of Xiawan(CV 10) was higher than Qihai (CV 6) as well(P<0. 01). (2) MBPU values of Danzhong(CV 17), Xiawan(CV 10) and Qihai(CV 6) were increased apparently compared with those before treatment after EA stimulation at Zhongwan(CV 12) for 20 min in the two groups(all P<0. 01). (3) The rise rates of MBPU level about Danzhong(CV 17) and Qihai(CV 6) in the yang-deficiency constitution group were higher than those in the mild constitution group without statistical significance after EA at Zhongwan(CV 12) for 20 min(both P>0. 05).</p><p><b>CONCLUSION</b>The energy metabolism in CV of volunteers with yang-deficiency constitution is declined, especially Danzhong(CV 17). EA can rise energy metabolism in CV of mild or yang-deficiency constitution volunteers through regulating MBPU along meridian.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the needlestick injuries in student nurses during nine months of in-ternship in our hospital, and reveal the high-risk periods, risk procedures, and influencing factors for needlestick injuries, and explore the prevention approaches.</p><p><b>METHODS</b>Three hundred and fifty student nurses who interned at our hospital from April to December 2014 and from July 2014 to March 2015 were surveyed using self-de-signed questionnaires. Three hundred and forty questionnaires were recovered and 334 out of them were valid. Data were collected and questionnaires were analyzed.</p><p><b>RESULTS</b>The incidence of needlestick injuries was 60.8%; the incidence of needlestick injuries was substantially higher at the early stage than at the late stage of the internship, and higher in the day shift than in the night shift. Moreover, the incidence of needlestick injuries was the highest during the removal of a syringe or infusion needle, accounting for 24.3% of the total incidence. Some other significant factors for needlestick injuries in student nurses included education level, reports on oc-cupational exposure, constant update of nursing knowledge, regular hematological examination, and relevant training experiences. According to 61.7% of student nurses, clinical operations were affected due to underlying concern about needlestick injuries.</p><p><b>CONCLUSION</b>More attention should be paid to high incidence of needlestick injuries in student nurses, especially at the early stage of their internship. To reduce the incidence of needlestick injuries, education on occupational protection should be given to student nurses in advance, and the pre-job training should be enhanced.</p>

ABSTRACT

Objective To investigate the feasibility of Six Dimensions Scale (6-D scale) of Nursing Performance used for evaluating the effect of standardized hospital-department training on new graduate nurses.Methods 6-D Scale of Nursing Performance was conducted to evaluate the nursing ability of 216 new graduate nurses (2013) of our hospital pre and post the three-month intervention,including the self-appraisal and appraisals by head nurses.Results The nursing ability was improved significantly after the training.The total score of self-appraisal 6-D scale were significantly higher than that before the training (142.34±19.92 vs 135.81±18.30),in which the subscales scores within the dimensions of leader-ship,critical care,teaching/collaboration,planning/evaluation,interpersonal relations/communication were increased significantly,while the increase of professional development was not significant.Six-D Scale and six subscales scores based on self-appraisal were higher than that based on the head nurses' appraisals.Conclusions The application of 6-D scale is scientific to evaluate the effect of standardized nurse training.We had demonstrated that standardized hospital-department training is of great benefit for new graduates to improve the nursing ability and adapt to the clinical circumstances.

ABSTRACT

Objective To screened out items with large proportion of technical analysis in obstetrics and gynecology physical examination by using statistical parameters Kappa index and to evaluate effect and clinical significance of Kappa index based on objective data and students' and teachers' subjective feeling.Methods We made Kappa index screening with double-blind method and established the Kappa index table for teachers and students.Students were randomly divided into conventional teaching group (n =26) and teaching reform group (n =29).Students in conventional teaching group traditionally followed the teacher to carry on the practice while those in teaching reform group received training according to the numerical value index and had examination after training.We paid attention to each student's weak points,provided personalized guidance and communicated with students actively.Results We found that it was uneasy for beginners to quickly grasp four step obstetrics palpation in-spection according to Kappa index.There were statistical differences in operation of gynecological physical examination between two groups (P =0.016).Conclusions Application of Kappa index can help us screen out skills in the operation suitable for training for eight-year program medical students.With quantitative evaluation,we can improve quality of clinical practice teaching in department of obstetrics and gynecology and guide educational reform.
